Device FAQ: How do I Return a Device?

What condition is your device in?

Choose the answer that best describes the devices current state.

  1. Complete the green return form with the following:
    1. Your Name
    2. Company Name
    3. Site
    4. Address
    5. City, State, Zip
    6. Check: "Sensors Unused"

See picture below for an example:

2.  Remove the antennas from the device (if applicable)

3.  Package the materials using the inserts provided.

4.  Place the green return form in the box.

Ship all devices to:

Advanced Technology Services, Inc.

  1. Complete the green return form with the following:
    1. Your Name
    2. Company Name
    3. Site
    4. Address
    5. City, State, Zip
    6. Check: "Sensors Used".

4.  Place the green return form in the box.

Red repairable part tags will be included in all install kits and will be added to all replacement device requests.

1. The following information is required on ALL repair tags:

  • Name (of the person returning the device)
  • Company (name of company device is coming from)
  • Phone Number
  • Problem Description

 

2. Tie the red tag to the core of the device.

3. Remove the antennas from the device (if applicable)

4. Package the materials using the inserts provided.

5. Place the green return form in the box.
